Oswego Hospital will offer a first-dose vaccine clinic from 8 a.m. to 2 p.m. on Friday, March 26. 

Hospitals are now able to vaccinate anyone eligible under New York State guidelines. To check your eligibility, visit the "Am I Eligible?" website. The clinic will be held at the Oswego Hospital's JPC conference room, 110 West Sixth St. in Oswego.

All individuals receiving the COVID-19 vaccine must complete the New York State COVID-19 Vaccine Form for the first dose, pursuant to Executive Order 202.86, and attest that they are eligible to be vaccinated, Practices, providers and entities must confirm adherence to this requirement at the time of vaccine administration. Individual should be able to show they have completed this state vaccine form prior to receiving their vaccination. This vaccination is scheduled with the Moderna vaccine.

All individuals must produce proof of eligibility at time of vaccination. Such proof may include a letter from a physician specifying co-morbidity or the following:

  • an employee ID card or badge
  • a letter from an employer or affiliated organization
  • a pay stub, depending on the specific priority status
