Second COVID vaccine clinic set for students: Friday, April 9
Dear SUNY Oswego Students,
SUNY Oswego is pleased to announce that we have secured an additional 500 doses of the single-dose Johnson & Johnson vaccine to administer at a second on-campus vaccination clinic for all students (on-campus, off-campus and Syracuse campus students).
Appointments will be granted on a first-come, first-served basis so please make your appointment via the hyperlink below as soon as you are able.
Please note the following requirements:
- Prior to your scheduled vaccination, please fill out the following forms:
- New York State COVID-19 Form – Take a screenshot of the verification page showing you’ve completed this form or print and bring to your appointment.
- COVID-19 Immunization Screening and Consent Form - Please print out and bring this completed (mandatory) form to your appointment.
- Students must be 18 years or older to be eligible for this vaccine.
- Please bring your SmartPhone (if applicable), SUNY Oswego College ID, and proof of birth date to your scheduled appointment.
- Please bring your insurance card (Note: you will not be charged for the vaccine).
- If you have already received the first dose of a vaccine, DO NOT schedule a vaccine appointment for this clinic.
- If you attempt to sign up for a vaccination and all time slots are full, please add your name to our COVID-19 vaccine waiting list by completing this Google form.
We strongly encourage our students to get the COVID-19 vaccine, especially members of the graduating class of 2021 who must show proof of vaccination or negative test within 72 hours of their Commencement ceremony.
